Do I need a doctors’ referral?

A doctors’ referral is not required to see our Sports Medicine Physician or any of the therapists at High Performance Sports Medicine. However, if you would like your family doctor to receive a narrative report about your initial sports medicine consultation and important updates during the course of your care, then we would suggest that you have your doctor provide you with one. Also, your health insurance provider may require a doctors’ referral to reimburse you for services provided by one of our tharapists or for bracing and custom orthotics.

Are these services covered?

Appointments with our Sports Medicine Physician are covered under the Ontario Health Insurance Plan. A valid health card is required to see the Sports Medicine Physician and must be brought to each and every visit. It is your responsibility to ensure that your OHIP card is valid.  If your health card has expired please contact the Ontario Ministry of Health and Long-Term Care for its renewal. Residents of Quebec as well as non-residents of Canada are required to pay for their visit with the Sports Medicine Physician.

Physiotherapy, Chiropractic Care, Registered Massage Therapy, Acupuncture, bracing and custom orthotics are covered under most health insurance policies, however some policies require a referral from a doctor prior to claiming reimbursement for these services.

What are the payment options?

Physiotherapy, Chiropractic Care and Massage Therapy appointments require payment at time of service. Payment can be made by cash, debit, VISA, MasterCard or American Express.Personal cheques are not accepted.Unfortunately, we are unable to directly bill your insurer.

What to expect on your first visit?

On your first visit to High Performance Sports Medicine you will be warmly greeted by our receptionist and asked to complete a Clinic Registration Form & Medical History Questionnaire. Depending on which practitioner you are seeing it may be necessary to complete additional forms. Please arrive 10 minutes early to your first appointment to allow adequate time to complete these forms. Should you wish to complete these forms prior to your first visit please go to myhpsm where you will be able to download the appropriate forms online.

How should I dress?

To allow for proper assessment and treatment please bring loose fitting pants, shorts or a tank top to allow for proper exposure of the area you are being seen for. If you are seeing a Massage Therapist your treatment will take place in a private treatment room and you can discuss with the Massage Therapist prior to your first appointment which areas will require exposure. Please bring along your running shoes, orthotics and braces so we may assess their condition and appropriateness for you.

How safe is electronic charting and how safe are my personal health records?

The privacy of your personal and health information is extremely important to us. High Performance Sports Medicine adheres to the principles outlined in the Canadian Personal Information Protection and Electronic Documents Act. As well, Health Screen Solutions Inc., our electronic service provider, is dedicated to upholding your privacy and confidentiality.  They requested an informal review from Ontario’s Information and Privacy Commission and were found to have ‘strong safeguards in place’.
